Your First Visit Starts Here
We want your first visit to feel simple and straightforward. Find everything you need to prepare for your appointment, complete new patient forms, explore payment options, and learn what to expect before you arrive.

What to Expect as a New Patient
Interested in Our Advanced Knee Pain Program?
Patients interested in our Advanced Knee Pain Program have the option to begin with a complimentary consultation with a provider.
This optional consultation gives you an opportunity to learn more about the program, ask questions about the services available, and determine whether you’d like to move forward with an initial exam.
The complimentary consultation is not a medical exam and is not required before beginning the program. Individual treatment recommendations are made after your initial exam and review of your medical history, condition, imaging, and examination findings.

Before Your Visit
To help your first visit go as smoothly as possible:
- Complete any requested new patient paperwork before your appointment.
- Bring your photo ID and insurance card, if applicable.
- Bring or provide any relevant medical records or imaging you’ve been asked to supply.
- Bring a current list of medications and supplements, if requested.
- If your paperwork is not completed in advance, please arrive at least 30 minutes early to allow time to complete it before your scheduled appointment. If there is not enough time to complete your paperwork before your appointment begins, your visit may need to be rescheduled. This helps ensure your provider has the full amount of time needed to give you thorough, attentive care. If you have questions about which forms you need for your appointment, please contact our office.
Payments & Insurance
Payment and insurance coverage can vary depending on the service you receive. Some services may be billed to insurance, while others may be offered on a self-pay basis or as part of a program.
When applicable, our team can help you understand insurance coverage and your financial responsibility so you know what to expect before moving forward.
Payment and financing options may also be available for qualifying services.
